The convex equilateral form with regular polygonal faces is the Johnson solid J_(57).

It has 15 polyhedron vertices, 30 polyhedron edges, and 17 faces. The faces consist of 12 equilateral triangles, 3 squares, and 2 regular hexagons (McCooey).
