Generalized Amplitude-Damping Channel

A generalized amplitude-damping channel is a quantum channel on a qubit that relaxes its excited-state population toward a specified thermal value. For damping strength gamma in [0,1] and stationary excited population nu in [0,1], it acts by

 A_(gamma,nu)([1-q z; z^_ q])=[1-t sqrt(1-gamma)z; sqrt(1-gamma)z^_ t],

where t=(1-gamma)q+gammanu. The input density matrix requires 0<=q<=1 and |z|^2<=q(1-q). At gamma=0 the map is the identity; at gamma=1 the output is the fixed diagonal state with population nu.
